| Performance Metrics: | Ninety 90% of sexual assault survivors identified, will receive at least two(2) duplicated or unduplicated services from the 7 options listed under the scope of work which include Advocacy, Accompaniment, Crisis Intervention, Information and Referral Services, Support Group, Therapy and Forensic/Medical examinations. The two services could be 2 separate accompaniments or 2 different services. We expect that approximately 1,500 survivors will receive 17,000 services. In the current year 2020-21 we expect approximately 1,500 survivors will receive 17,000 services as well. | |
| Financial Consequences: | If the PROVIDER fails to achieve the minimum monthly deliverable in accordance with the agreement, the PROVIDER will be assessed a financial consequence in the amount of 3% of the total monthly reimbursement request. | |
